I went through this sphincterectomy once and may need it redone for a small section, but yes am in the same situation, except in my case the sphincter was the real problem and not the bladder, so for me intermittent catheters was no solution. They suggested a continence urostomy but I refused this once I heard reviews from patients in that hospital that gave me the pros and cons. For me the 12 o'clock sphincterectomy was a very good decision. While I leak I can still feel the sphincter contract heavily but it no longer causes retention
  2. Trust me it won't work if you have no underlying urological or neurological problems. I discussed this with a very cooperative urologist who I was seeing for DSD (sphincter issues) and because at first invasive procedures were out of the question I discussed catheters, he said that while it would temporarily solve the DSD, it would not be permanent and the risk for infections great. In the end a sphincterectomy was decided to be the better alternative because I really didn't want a urostomy.
  3. Those spasms are indeed the worst, if they don't get better after a day or 2 then it won't ever. I wore them with sterile drainage bags for 2 - 3 weeks each until it got more painful and I couldn't keep one in longer than a few days. I used catheters intended for 4 to 6 weeks with special coatings and silicone ones but it didn't help After 4 months I had to stop. The spasms wouldn't decrease even with medication It's important to let your body heal and recover when you had much discomfort with a catheter, not doing so will cause infection and damage over time.
  4. I was charged for 4 months and it did increase the urge but not the capacity. Once the sphincter clenched again, they drained more than 300ml , even now that they cut a part out of the sphincter I still feel pressure buildup while lying down or sitting and flood when standing and moving shortly after.
  5. The untraining program states to drink more so you relax the sphincter more often to go. It's not focussing on shrinking the bladder yet. After months of very frequently voiding you get into the automatic voiding pattern and develop an overactive bladder. Then you can normalize liquid intake to the 2 liters a day. In time your bladder nerves will respond to void so soon you'll think your bladder shrunk massively. In reality this muscle rarely shrinks, but it's the nerve impulses that adapt to void asap. This latter stage can take years for some, months for others.
  6. As I have work Foley catheters for 4 months I can provide accurate feedback. Yes you bought a catheter too small. Your information about 40cm length is correct, some come in 41cm too. Thickness: for men the average is 12 to 14 Fr/CH , anything thicker will definitely hurt and damage tissue if you have not done this or had one frequently The pain in the center of the penis you describe is irritation near the bladder neck (entrance of the bladder) which has nerves going all the way up to the penis shaft. (My urologist explained this to me as I had similar complaints. It should go away after removal after 24 to 48 h, longer than that may indicate injury or infection The incontinence after removal is normal but temporary. After a while everything will revert, even after many months of wearing one , though it will take longer. During insertion it is imperative you work sterile as possible. Disinfect hands, genitals, foreskin before starting. Use sterile lubricant or preferably instillagel , which contains lidocaine to reduce pain. From the picture it seems your catheter is not with a balloon so it cannot be kept in, unless I didn't look properly, and then you're using an intermittent catheter. Those cannot remain in place and should be removed after the bladder is empty. when you consider using Foley catheters, UTI's are inevitable even if you work clean, if you have an active lifestyle you increase irritation and infection rates. It's true that nurses insert them too rough, one scraped a chunk of flesh from my bladder neck away and that hurt for weeks. Catheter material is also important. The cheap yellow silicone coated latex catheters are fine for a few days max but irritate a lot. (Rusch gold). The silicone elastomer coated latex caths are for me ideal, very soft and flexible but cost more. (Bard Silastic, rusch silasil plus), these are 4weeks usable. The pure silicone ones are what hospitals recommend but they are so tough and hurt, I had much pain when walking with those. , They're very expensive and last up to 6 weeks Even so having a catheter without attached bag inserted for more than a couple of minutes is dangerous for bacteria to enter the bladder and cause a UTI. That's why intermittent catheters must be removed as soon as you're done. I'm glad I'm rid of all that now. The last one I had in me was for a sphincterectomy surgery.
  7. They never gave me an official name, officially though it's called a sphincterectomy but it can be done in so many ways. I asked to do a partial excision of the urethral sphincter. And so the doc said "so... basically you want me to cut you incontinent?" And I confirmed. Given the history of treatments and problems he agreed to cut out a small V section on the upper side of the sphincter but not on the lower side and not a wider part because he'd jeopardize his career otherwise but it definitely did the trick!
  8. I too experienced this, having a shy bladder myself I was never able to properly untrain. Though there were sphincter spasms and over impulsed nerves involved due to a turp surgery, which made it even harder. Not even high doses of several types of muscle relaxers or Botox would help so after years of struggling and even being forced to use Foley catheters for months I was finally granted my wish by cutting out a small piece of the urethral sphincter. Now it can relax better but I still noticed it spasms but I leak regardless. I find it very special feeling not to have to worry about relaxing and not having full control. But in all those years nothing in the 12 month program worked for me. And neither was the medical world. So I was desperate and sick of it, and now I'm finally at peace.
  9. I definitely have spasms when dehydrated, but in my case it is the sphincter not the bladder, because the wound where they cut a piece out of is irritated then because of the concentrated urine. My bladder only spasmed when I had Foley catheters in but now it doesn't anymore. The urethral sphincter however continues to spasm heavy and painfully but at least I leak through the hole they cut out.
  10. Yup, I know that problem. After my sphincterectomy I had similar amount of blood loss, same after my turp. Sphincter is still healing, almost 2 months after surgery but bleeding stopped after 3 weeks. I'm still incontinent so that means it's not growing back together which is good

Since I'm a diabetic too I once was wondering if I would get that too but unfortunately it's only caused by high levels of HbA1c and this often causes damage to other nerves (feet) as well so since I always try to keep mine at 6.5% I realized I'd never get such bladder neuropathy. I wouldn't even want to risk endangering my health for that either. So I had no choice but to find other ways. Now after the first surgery of the urethral sphincter I must say I'm really happy I found a doctor that listens and helped. I'm finally rid of my detrusor sphincter dyssinergy, at least in the upper half. And can empty it again, albeit without control. Changing, cleaning yourself and showering can prove a challenge but it's well worth it Just the intense post surgery pain is a bummer but once that's all healed up I'm right as rain. I can't wait till the other half is done so I get my sleep back because now I wake up every hour from heavy urge, but then I turn over and it leaks out. It felt so good to say goodbye to catheters!
  12. 15 years of digging up the entire internet, asking around, going to 5 urologists, you learn in time. Nerves are also a sensitive part, once you learn to listen to them, you getto know them better, perhaps too well. but yes, most i learned from digging into scientific research documents online and from other people who had this surgery before too. In my case it all began with prostate issues though (BPH) , the TURP surgery damaged a nerve in the sphincter. If this surgery could have been avoided or was not needed (but unfortunatly it was) I would never be in this position

I had pretty much every size inside me, from CH12 to CH 30 (the bigger ones in hospital btw) I can tell you that for comfortable daily long term use, ch12 and 14 work best when you are a mobile person. of course, the thinner they are the sooner you have to replace them because of the bacterial biofilm buildup (encrustation) but a ch14 is definitly the best choice for most people who still need to walk around and do stuff. anything thicker caused urethral trauma in my case, when wearing them longer than 48h without severe painkillers the CH20 and up were inserted by surgeons for blood and tissue particles after prostate, bladderneck and urethra surgeries, so those sizes should never be used for normal daily activities.
  14. Oh i have no goal of regaining control at all. I prefer leaking above muscle complications. He'll have to cut a wider section out of the upper half because it is getting a bit stiff from possible scar tissue and do the same on the lower half. This way I will be completely unable to fully close the sphincter. I really was getting tired of these muscle spasms and now when I am voiding, both controlled or not, I can feel that the upper half is no longer contracting while the lower one is painfully overcharched (nerve impulse) and contracting heavily.
  15. Well the surgery went really well! They cut a V section out of the upper half of the sphincter, I had little blood loss and now no pain. I dribble or leak small amounts every time I move, but the bladder can still hold quite a bit, he said he wanted me to have a chance to still retain some continence. But because I was so pleased with how it went I insisted on having the other side done later. He said "fine by me, you're the boss" xD
