I wrote a story called the toilet permit (I think it's kicking around here) about a boy and girl who had to go back through toilet training (with their learners permit).

I also wrote the Restricted License story based on these birth announcements:

You were a bit late on that. When I was in Jr. High in the 1956-9 timeframe. If you were to go to the toilet, you needed a slip called a "Basement Permit" signed by the teacher heading your class at the time. This provided specific notice that you were clearted to be out of class

We called them "hall passes."    Some were on a "official" printed form made out in your name.   If you just needed to use the bathroom, each room had a wooden pass as well.

In high school print shop we enlarged the hall pass form and made a pad of them about 4x the original size as a joke.
