Here's something I discovered that I haven't seen elsewhere: I've started wearing a "BIKE" brand of compression gym shorts over my diaper (I usually wear Attends or Tena or Molicare for day) and it really helps a lot. So the layers are diaper, BIKE shorts, pants during day...and at night I usually go with a Molicare "super", plastic pants, and BIKE short over that.

The compression shorts really help to keep the dipe in place and goes a long ways to help prevent gaps and leaks, at least in my case. It also helps cut down on noise a bit, which is a bit uncalled for at work, I think.
