A really comfortable, soft and silent diaper cover on Amazon for under $20. Cloth lined so it can be worn as underpants or as added protection when wearing a guard or pad. Wide-bottom made for going over a diaper. I'll order more when I finish my diet and know what size I'll need long-term (Hopefully I can do it this time!)
