Xyxy, well I have to say it certainly does sound like what I am going through right now. I have not had a fluoroscopy, but think i should now. During my last urodynamics test I had an extremely difficult time peeing past that small catheter, and had pressure in my bladder that was literally off the charts. I will definitely try and get my urologist to put me on prozasin now.

One thing that I haven't understood lately is the bladder spasms I've been getting lately. They can occur when I have almost no pee in my bladder now. As an example, I have to catheterize, usually for about a week once a month. This is to give my system a chance to heal from my constant straining. Right now I am openly draining into my diaper, yet about two or three times a day I am still getting urges. This has not happened before, and I thought it might have something to do with this last botox injection I had gotten. Did you ever get urges while catheterized?

I too believe I had spinal damage, thought I never was able to get this officially diagnosed as what's causing my bladder problems. Still, I can definitely say that over the years my problems have been getting worse. I though it was because of the many different attempts to "cure" me of my incontinence were cumulatively making it worse. I wonder now if the "professionals" have just been ignoring what I've been saying, AND looking in the wrong place all along.

I've got about another month before this botox injection in my sphincter wears off, and I see my urologist again. When I do see him I will request the prozasin and a fluoroscopy. Thanks for the advice!

Well did what I said I would never do again..... I had a lower dose of botox done and this time I'm really paying for it... It has been about a month and I have gone from being able to pee, with the a few accidents a day to now not being able to empty my bladder... I still have accidents but now they are smaller . Did my check up and the doctor took one look and said that I have a UTI, no test, nothing.....

I know that I said this in the past, but never will I do the botox thing. I can not do it, I hurt to pee (but my doctor says that is UTI) I have no stream, and I can only empty about half my bladder.

Well I got the botox injection done today. So far I actually feel no different- other than a little more sore in my bum. Just to recap, the injection was for trying to heal a bleeding fissure that was caused by a really bad hemorrhoid which did not heal properly. Well, it turns out the doctor said I was already developing another hemorrhoid again and wants me to go see a specialist about getting them so often. Well duh- you think.

At least it is a procedure they know how to do, since it is done pretty regularly, unlike the sphincterotomy.

True, but I've learned with doctors they are extremely reluctant to do two things at once, and I definitely am still fighting to get them to do the sphincterotomy. Having my prostrate also removed at the same time is honestly not a priority and would likely just complicate things.

Like I said though, might still be worth bringing up.

This would be done instead of the sphincterotomy. With a radical prostectomy, the bladder sphincter is removed along with the prostate. Incontinence is a frequent side effect, unless the individual is able to train the external sphincter. With the reduced constriction of the urethra through the prostate and the internal sphincter, which in most cases is the primary control for continence, you may be able to urinate more easily or be incontinent. Either way, your straining problem would be solved. I'm not a doctor. I'm just saying it might be an interesting alternative to discuss, since they seem to be adamantly against a sphincteotomy.

Not instead for me. I cant leave my not being able to pee up to a potential side effect. As best as I can tell my problems are coming from my external sphincter not opening as or when it should. As such I would need to have both done to ensure I no longer need to strain to pee.

No problem. The botox I had injected into my anal sphincter did absolutely nothing for me, except give me a case of gas incontinence. I simply wasn't able to hold back a fart. Go ahead and laugh, that part was funny- even to me. It has about completely worn off now, but I still have anal bleeding so my general surgeon doctor has referred me to a colo-rectal specialist. I should be making that appointment within a few days, but I already know the next step is surgery to close off the fissure. Obviously my underlying problem is still from the urology side though.

As I stand it has still been under three months since my last botox injection into my urethral external sphincter. Any benefits it had wore off about a month ago though. I was to be scheduled for another follow up visit with my urologist's partner (who did the actual botox injection), but it turns out he was just going to do it again. I protested, citing that if it didn't work good enough the first time why try it again while expecting a different result. That is the literal definition of insanity- which I am not.

So now I have been referred back to my primary urologist, and have an appointment with him in a couple of weeks. I've thought about and researched the possibility of me having a bladder neck narrowing, but I do not believe this is my problem. Often I am able to pee ok after I have started going. I am also able to empty my bladder, though with difficulty. Also, I am still having to catheterize every few weeks to give my system a chance to recover some. When inserting it I have an incredible amount of resistance as I try and get it past my sphincter (the pressure of which I can absolutely feel and tell is my sphincter- not the urethra or prostrate). As such, I do not want to muddy the waters with exploring this route. I will instead continue to peruse the sphincterotomy.

I'll give another update when something actually happens, or dare I say change for the better.
