This is the original of the RUFFLES & RIBBONS GIRLS' HOME BabyDoll diaper. It was created in 1951, by a lady who worked in a dress shop for her 5 year old little girl who was a bedwetter using the just released pre-fold diaper, it is NOT a baby diaper but made for long term use between the ages of 5 and 10 or more, being adjustable over a large range of sizes. It is the precursor to, or proto contour diaper

It was made by attaching two 14 x 21 at the end and then attaching the end of the third one at the midpoint

it is used by laying out, folding the back so that it will fit in the largest pair of baby rubber panties, usually something like "Big Baby", "Super Large", "Special Size" or "Toddler plus", or if you could get child size rubber panties. The "stem" is pulled between the legs, the arms wrapped around and made snug then pinned in the front  center. It might be considered a primitive type of "contour" diaper.
