There are things which go along with transitioning TG's- binding, breastforms, and so on. You worry about such things because they are important. But if you're worrying about your Doctor, then you have the wrong Doctor. If a care provider can be OK with TG's, I doubt that diapers would much matter to them unless you were wearing them for untreated incontinence. For the MtF, if you are having any incontinence issues, the doctor prescribing the hormones might want to lower your dosages of spironolactone, until you get examined by the urologist.
