I would guess that the best way to explain choosing not to have surgery, besides the fact that it is difficult, expensive risky, and permanent, is that very little of our gender happens in the act of sex between two people, and the act of sex itself goes on largely in the mind with a few cues from the skin.

So what is or is not found between a perso's legs simply isn't the most important component of finding the proper gender for everyone. It is a lot more about being comfortable in one's head and one's social relationships.

Why would someone not want the surgery?

The same reason some people chose to grow their hair and others don't. The same reason some of us like cats, some like dogs and some like neither. Everyone is different. Some boys have a vagina. Some girls have a penis. It's not a big deal. It's really not.
